About the Program
The care management program is a collaborative, person-centered service that focuses on the coordination and management of someone’s care. We use a holistic approach to ensure individuals and families are always supported.
Components of Care Management
- Assessment: Identify the individual’s baseline status, support needs, current supports, available resources, goals, and potential barriers to accessing support.
- Care Planning: Create a written care plan that identifies short-term an long-term support needs, goals, and stakeholders. The care plan outlines an action plan for coordination and monitoring and is updated as needs change.
- Coordination: Identify and coordinate services/transportation; Make referrals, schedule services, attend meetings/appointments; Keep stakeholders informed to assist with decision making.
- Monitoring: The Care Manager monitors the safety and well-being of the individual, quality of care from support services, monitors for changes to the individual’s baseline status, and ensures support needs are met.
